Player Registration Application
Player Freeze ApplicationPlease complete all the information on this form. Freeze (pre-selection) requests must be submitted to the Director of Coaches, dir.coaches@frysc.org, by the last day of regular registration.
Player Play-Up ApplicationA play-up request must be submitted by the last day of registration (before the start of late registration) with the previous coaches recommendation. The player must then go to a rating session for the age group that he/she wishes to play in. The Board of Directors reviews all requests and notifies players if the play-up is granted.
Player Request ApplicationThe Team Formation Committee shall consider special requests for placement of players with another player or a coach. Conversely, this form may be used to request that your child is not placed on a certain coaches team. Parents/guardians must send this letter to the Team Formation Committee NO LATER THAN May 11th in order for the request to be considered. Please send to FRYSC Team Formation Committee, 369 S. Coaching ApplicationPlease complete all the information on this form. Coaching requests must be submitted to the Director of Coaches, dir.coaches@frysc.org, to be considered.
Administrator Registration Application
Administrator Live Scan ApplicationPlease complete the following form and submit it at the time of your finger printing. The form includes a CalSouth code that ensures it will be distributed to the appropriate persons.
