Administrator Registration Application
Coaching ApplicationPlease complete all the information on this form. Coaching requests must be submitted to the Director of Coaches, dir.coaches@frysc.org, to be considered.
Player Freeze ApplicationPlease complete all the information on this form. Freeze (pre-selection) requests must be submitted to the Director of Coaches, dir.coaches@frysc.org, by the last day of regular registration.
Player Play-Up ApplicationA play-up request must be submitted by the last day of registration (before the start of late registration) with the previous coaches recommendation. The player must then go to a rating session for the age group that he/she wishes to play in. The Board of Directors reviews all requests and notifies players if the play-up is granted.
Player Request ApplicationThe Team Formation Committee shall consider special requests for placement of players with another player or a coach. Conversely, this form may be used to request that your child is not placed on a certain coaches team. Parents/guardians must send this letter to the Team Formation Committee NO LATER THAN May 11th in order for the request to be considered.
